(NOTE: These forms are for downloading by pastors, their representatives and church staff. Individuals interested in church annulments should meet with their pastors for forms. The Tribunal will not accept a Petition for Church Annulment from an individual. The pastor or his designee must be a part of the process and initiate the process on the Petitioner's behalf.)
